Registration that a CEA inspector would recognise

Name, sex, date of birth or age parts, mobile, address with GST state code. Optional ABHA, shown, not used as the primary key. Aadhaar is not a field.

Home collection

A slot is not an order. Marking collected mints or attaches the order so the bag has a barcode. The phlebotomist PWA is the same app, narrow.
